As part of the WIMBA device fitting process, it's crucial to ensure that pet parents are aware that WIMBA orthotics are medical devices and agree to their proper use. To facilitate this, we've created a Pet Parent Agreement document that should be signed by the pet owner prior to ordering a device. 




Steps for WIMBA Providers:

  1. Download the Pet Parent Agreement

    • Ensure you have both printed and digital versions available for convenience

2. Prior to ordering a device:

    • Present the Pet Parent Agreement to the pet parent.
    • Explain the importance of the document and what it entails.
    • Have the pet parent read, understand, and sign the agreement.

3. After Signing:

    • Scan the signed document. A simple photo of the printed page with the pet parent's signature is sufficient.
